Student Health Records
For the health and safety of our campus community, the Student Health Center must have an up-to-date health history for each student that includes information such as food/drug allergies, medical conditions, medications, etc. All students may submit health information to the Point Park University Student Health Center through a single electronic Student Medical Report Form. Health documentation requirements vary based on a student’s housing status.
Who is required to submit health records?
Health documentation is required for undergraduate on-ground students.
- Undergraduate students living in campus housing must meet full immunization requirements, including vaccines required for residential students.
- Undergraduate students not living in campus housing must meet limited immunization requirements, as defined by university policy.
- Graduate and doctoral students are not required to submit health records.
Graduate and doctoral students may voluntarily submit health information to the Student Health Center. If submitted, those records will be maintained in the student’s health record. However, incomplete or missing documentation for graduate or doctoral students will not result in disciplinary action, registration holds, or compliance enforcement.
Submission of health documentation does not, in itself, establish a provider-patient relationship with the Student Health Center.
Important Information for Student Athletes
Student Health Center forms and Athletics medical forms are separate requirements. The Student Health Center does not complete, review, or forward Athletics forms, and Athletics forms will not be accepted in place of Student Health requirements.

How to Submit
All students must complete the Student Medical Report Form, which includes:
- Health history
- Emergency contact information
- Consent for care
- Needs assessment
- Immunization documentation upload
Students will upload their immunization records directly within the form. Students who are unsure of their immunization status should still submit available records. The Student Health Center will review submissions and follow up if additional information is needed. The Student Health Center does not accept physical copies of the student health records. Immunization Exemptions
Students requesting an immunization exemption must complete and upload the required exemption form as part of the Student Medical Report Form.

- The completed form must be uploaded to the Student Medical Report Form at the time of submission.
- Exemption requests will not be reviewed unless the completed form is submitted.
- Requests are reviewed by the University, and students will be contacted directly regarding next steps
Students with approved exemptions may be subject to additional public health requirements. In the event of an outbreak of a vaccine-preventable illness, students who are not immunized may be excluded from classes, housing, or campus activities in accordance with public health guidance.

Requesting Records
Student health records are kept for seven years from the date they become inactive. Inactive records result from graduation or withdrawal.
For any record requests, please download the Record Release Form. The form must be emailed in PDF format to studenthealth@pointpark.edu along with your full name, date of birth, and year of enrollment. Please allow 5 business days for your records to be located in storage. You will receive and email back to make arrangements for the delivery of your records.

- Athletics medical forms must be submitted directly to the Athletics Department
If you are a student athlete, you are responsible for completing both sets of requirements.
