As we approach the beginning of the Fall 2021 semester, and the August 1st COVID-19 vaccine deadline has expired, I am sending a reminder to all students regarding the COVID-19 vaccine mandate referred to in the emails sent to you back on June 29th and again on July 29th.

As a reminder, all students participating in face-to-face classes or campus activities during the 2021-22 academic year (vaccinated or not), are required to complete the COVID-19 Information Survey. This important health information form is required for all students in order to have access to campus. Students who do not complete this form by Wednesday, August 18, 2021, will no longer be permitted on campus, student ID access will be discontinued, and they will not have access to campus buildings and facilities.

Students with a valid medical or religious reason are permitted to apply for an exemption. Students applying for an exemption must also complete the survey referenced above. Exemption process:
  • Complete the COVID-19 Immunization Waiver Form
  • Provide the appropriate statement based on the type of exemption (see form)
  • Send the completed form and statement to the Student Health Center by email at studenthealth@pointpark.edu, fax to 412-392-3801, or mail to Student Health Center, Point Park University, 201 Wood Street, Pittsburgh, PA  15222
