Vaccine Information
Point Park University requires COVID-19 vaccinations for all students participating in face-to-face classes or campus activities. Vaccine information is reported on the Student Immunization Record form, which is part of the Student Health Record form. For more information on student health records, visit the Student Health Center page.
Students are also required to complete the 2022-23 Covid Vaccine Data Collection Form. This applies to all students participating in face-to-face classes or campus activities for the 2022-2023 academic year. Note: If you provided this information last academic year, you are asked to provide it again this year.
Students with a valid medical or religious reason are permitted to apply for an exemption. Exemption process:
- Complete the Immunization Exemption Form
- Provide the appropriate statement based on the type of exemption (see form)
- Send the completed form and statement to the Student Health Center by email at studenthealth@pointpark.edu, fax to 412-392-3801, or mail to Student Health Center, Point Park University, 201 Wood Street, Pittsburgh, PA 15222
