PLAN DOCUMENTS
Whether you are already enrolled in Point Park University's student health insurance plan (SHIP) or deciding whether to enroll, these documents will provide important SHIP information such as benefits, out of pocket costs, etc.
2026-27 Benefit Book
The Benefit Book is the document that contains the terms of coverage for the plan. It explains in detail what the plan covers and what is not covered, along with all other plan-related information.
2026-27 Benefit Grid
The Benefit Grid is a chart that provides an overview of services covered under the plan, including the portion of the cost that is your responsibility. For a more detailed explanation of payment of covered services, please see the Summary of Benefits and Coverage (SBC) below. For more information about what the SHIP covers and does not cover, please see the Benefit Book above.
2026-27 Summary of Benefits and Coverage (SBC)
The Summary of Benefits and Coverage (SBC) shows how you and the plan share the cost for covered health care services in more detail than the Benefit Grid. It also includes examples of how payment and cost-sharing are applied in several scenarios. For the complete terms of coverage, see the Benefit Book above.
2026 Preventive Schedule
Preventive or routine care helps you stay well or find problems early, when they are easier to treat. As a part of your health plan, you may be eligible to receive some of these preventive benefits with little to no cost sharing when using in-network providers. All benefits for over-the-counter drugs and supplies must be purchased through in-network pharmacy providers in order to be covered.* Make sure you know what is covered by your health plan and any requirements before you receive any of these services. Recommended annual services are based on a calendar year, resetting on Jan. 1 of every year.
Some services and their frequency may depend on your doctor’s advice. That’s why it’s important to talk with your doctor about the services that are right for your age, gender and family history.
HELPFUL LINKS
UPMC Member Website
UPMC's member website has many helpful resources for members, including the ability to access:
- Finding in-network providers when you need medical care, such as a doctor, hospital or urgent care
- Pharmacy and prescription medication information
- Your virtual insurance card, where you can report a lost or stolen card, save or print a copy of your card, or request a new physical card
- Explanations of benefits (EOB), which is a document from your health insurance company that details how a claim was processed. EOBs explain what services were received, the costs associated with those services, how much the insurance company paid to the medical provider, and what amount, if any, you may owe. An EOB is not a bill, but it can help you understand a bill if you receive one and verify that the medical provider is billing you accurately.
- Mental health support
- Links to download the UPMC Health Plan app, access to health & wellness podcasts and more
Once your enrollment is confirmed and you have received your insurance card, make sure to register for a UPMC Health Plan account so you can access all of the above and more. If you are having trouble registering for a UPMC Health Plan account, reach out to Gallagher's Help Center for assistance.
